  • Schedule of Benefits
  • Census – gender, date of birth, Dependent Election*, zip code
  • Current Rates / Booklets (if applicable)
  • Three Years of Claims Experience for groups of 100 or more employees

*Dependent Election – EE Only  /  EE&SP  /  EE&CH  /  FAMILY

 

Schedule of Benefits:

Percentage of Coverage for Preventive, Basic, and Major Services
  • Preventive – 100%   /  Basic – 80%  /  Major – 50%
  • Employer can customize coverage percentage for each level of service
  • Employer can customize covered services

Deductibles

  • $25 Deductible applicable to Basic and Major Services
  • $50 Deductible applicable to Basic and Major Services

Benefit Maximums

  • $1,000 Annual Max
  • Employer can choose the annual benefit max

Orthodontia Coverage

  • Typically has a $0 Deductible / 50% Co-insurance / $1,000 Lifetime Maximum
  • Employer can choose whether or not to include orthodontia coverage
  • Employer can elect a higher or lower Lifetime Maximum

Contributions

  • Employer Paid – the Employer pays 100% of the premium
  • Contributory – Employer and Employee share in the cost of coverage
  • Voluntary – the Employee pays 100% of the premium

  • Employees with dental coverage are more likely to seek preventive care resulting in less time away from work due to dental issues
  • Visiting  a dentist can help detect other more serious health issues
  • Dental services are expensive when 100% of the fee is coming out of your own pocket
